What is Hospice

Hospice is a special way of caring for people who have a limited life expectancy, and for their families. We at Grand Terrace Hospice work with patient’s physicians and practice a palliative care approach in rendering the physical, emotional and spiritual support to patients, families and their loved ones. Our emphasis is placed on quality of life rather than length of life. Hospice services are available 24 hours, 7 days a week.


The Grand Terrace Hospice interdisciplinary team is committed to ensuring that patients are given the latest and most innovative approach and guarantee that patients spend their final days with dignity and the utmost comfort.

The care provided by Grand Terrace Hospice may be in their home or in a care center, skilled nursing facility or inpatient facility.

Our Palliative Care Service

  • Early and efficient discharge planning

  • Effective care management practices (to ensure medically and appropriately indicated durable medical equipment)

  • In-depth discussion of end of life care (to determine appropriate code status including POLAT and hospice referral if indicated to minimize hospital readmissions)

  • Consistent coordination with primary care physicians and medical groups (to achieve consistent post discharge follow-ups)

  • 24-hour physician or NP/PA coverage (to handle issues that arise in facilities rather than emergency rooms)

  • Actively manage the patients (to improve the group’s SNF bed days)

  • Conduct family meetings (to educate family on treatment options and progress)

Who is Eligible for Hospice Care?


Hospice care is a special benefit under Medicare (hospital insurance). You are eligible for Medicare hospice benefits when:


  • Hospice is covered 100% by Medicare and Medi-Cal. Benefits include all approved medications, medical equipment, oxygen and medical supplies delivered to your door.

  • Your doctor and the hospice medical director certify that you are terminally ill and have less than six months to live.

  • You sign a statement choosing hospice care instead of routine Medicare covered benefits.



Hospice Insurance Coverage

  • Hospice care is fully covered by Medicare and Medi-Cal benefits.

  • Most private insurance and managed care plans have a hospice benefit.

  • Care provided by the attending physician and treatment for conditions not related to the life limiting illness continues to be paid for by Medicare and Medi-Cal.

  • The patient or their representative can revoke Hospice to return to currative or treatment mode at any time. The patient may also reelect hospice care and move into the next period at any time.
